Fleetwood Town, working alongside Level Playing Field, would like members of the Cod Army to fill in the annual fan survey, which helps us hear the experiences of disabled sports fans.
The results will give us a clearer understanding of whether accessibility is improving for disabled fans or not and will therefore help us as a charity to provide better support - the survey will close on June 25 and the results will be released later in the summer.
The first set of questions are about yourself and attending live sports generally. After this are some questions specifically about the team you support, and then the option to answer questions for a second team.
