Fleetwood Town’s new Onward Card is now on sale to supporters.
Fans can purchase in the club shop at Highbury Stadium, over the phone by calling 01253 775080, or online HERE.
To complete your Onward Card application, supporters will require the following:
- Supporter number (if applicable)
- Full name
- Date of birth
- Email address
- Address including postcode and contact number
- Time at current address
- Previous address if lived at current address for less than three years
- Residential status
- Bank account number and sort code
- Bank name, address and postcode
